How to Find Your Immunization Records

Some students may need help locating their immunization records to complete Tulane’s immunization compliance requirements. One way to locate records is through a state immunization registry, also called an Immunization Information System (IIS). These are state databases that store vaccination records. Some states provide online record portals through their government websites, while others use systems such as MyIRMobile or the Docket app to provide access to records.

This resource is optional and is intended to help students locate prior vaccination records. Students must still submit their immunization records through the Campus Health Patient Portal as part of the immunization compliance process.

Important Note for Louisiana Students

If you received vaccinations in Louisiana, Tulane Campus Health can access records directly through the Louisiana LINKS registry. Most Louisiana students will not need to locate records through a state registry.

If you received vaccinations in another state, use the list below to find your state’s immunization registry and instructions for requesting records.

Before You Begin

Please keep these important notes in mind:

  • State immunization registries may not contain a complete record of every vaccine you have received. 
  • Some immunizations, especially those received many years ago or from providers who did not report to the state registry, may not appear in the system.
  • Some states allow online access to records, while others require a form, email request, mail request, or contact with a healthcare provider or health department. 
  • In some locations, records may only be released to healthcare providers, schools, or other institutions. 
  • If you cannot find your records in a registry, you may need to contact your previous healthcare provider, pediatrician, pharmacy, school, or local health department. 
  • It may take several days or weeks to receive records from some states.

State Immunization Registry Information

Use the information below to locate immunization records from the state where you received your vaccinations.

Alabama

Alabama does not offer a public immunization registry for individuals. Contact your healthcare provider or county health department for a copy of your records.

Colorado

Access your Colorado records through the online portal, or request them by submitting a notarized form.

Louisiana

Tulane Campus Health can retrieve Louisiana immunization records directly from the Louisiana LINKS registry. Most students who received vaccinations in Louisiana will not need to use a separate registry link.

Montana

Request your Montana records by mail, fax, or email. Records are only stored for individuals who opted in.

New York

New York maintains two immunization registries. New York City residents may access records online. Records from the rest of New York State may need to be requested through the state health department or local health department.

Ohio

Ohio immunization records are typically requested through a local health department or by mailing a signed authorization form to the Ohio Department of Health.

Puerto Rico

Puerto Rico does not offer a public immunization registry for individuals. Contact your healthcare provider for a copy of your records.

Rhode Island

Rhode Island generally releases records only to healthcare institutions, schools, childcare facilities, and similar entities. Students needing records sent to an out-of-state school can follow the contact instructions provided by the state of Rhode Island.

Texas

Request your Texas records by fax or email. Texas requires individuals or parents to opt in before information is stored in the registry.

If you cannot locate your records

If you are unable to locate your immunization records through a state registry, you may need to contact your previous healthcare provider, pediatrician, pharmacy, school, or local health department.
