Compliance Guidelines for New Students

The state of Louisiana and Tulane University require students to have proof of immunity for certain diseases. All students who have been admitted to Tulane University are required to provide proof of immunization compliance (meeting CDC and American College Health Association guidelines) prior to arriving at Tulane.

Tulane must have evidence of a student’s compliance with the state guidelines for immunizations. Failure to meet these requirements will result in an enrollment hold being placed on your student account, which will bar you from dropping or adding classes.

For new students, achieving immunization compliance requires three steps:

Step 1 - Submit a completed and signed Tulane Immunization Form.

The Tulane Immunization Form documents that the student has proof of immunity for the following diseases.

  • measles, mumps, rubella
  • tetanus, diphtheria, pertussis
  • meningitis
  • tuberculosis screening or testing (see form for details)

The student's health care provider will complete and sign the Tulane Immunization Form. Completed signed forms must be uploaded to the Patient Portal. Faxes and emails are not accepted.

Step 2 - Provide proof of receipt of the COVID-19 vaccine and recommended boosters per CDC guidance.

There are two ways to meet this requirement:

  1. Receive the vaccine and/or boosters via the Tulane Vaccine Clinic. (If you receive the vaccine and/or boosters through the University, you do not need to upload records to the Patient Portal because they will be automatically entered for you.) OR
  2. Receive the vaccine via and/or boosters via an external source. If you receive the vaccine and/or boosters outside of Tulane, you must submit documentation through the Patient Portal.
